Physical Activity Readiness Questionnaire  (PAR Q)

The PAR Q (Physical Activity Readiness Questionnaire) is required to be completed by each participant before taking part in training. 

For most people, physical activity should not pose any problems or hazards. PAR Q has been designed to identify the small number of people for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them.

Common sense is your best guide in answering these few questions in the next section. Please read them carefully and check YES or NO.  If YES, please explain. If you answer yes to any of the questions in the PAR-Q, you are advised to seek medical advise/approval before taking part. If your health changes, such that you answer yes to any of the questions, please refrain from exercising until you have spoken with your GP or other health professional. 

This information is not meant to make you feel anxious about training.  It is merely there to make sure we can give you the best experience possible and to keep you exercising safely.

​This form should be completed prior to your first session, OR if you need to tell us about any changes to your health which may affect your participation.

If you are aged 15-69, the PAR-Q will tell you if you should check with your doctor before significantly changing your physical activity patterns. If you are over 69 years and aren’t used to being very active, check with your GP. Please read each question carefully and answer honestly by indicating YES or NO.

Has your GP ever said you have a heart condition and that you should only do physical activity recommended by a GP?
Do you feel pain in your chest when you do physical activity?
In the past month, have you had a chest pain when you were not doing physical activity?
Do you lose balance because of dizziness, or do you ever lose consciousness?
Do you have a bone or joint problem (for example back, knee or hip) that could be made worse by a change in your physical activity?
Is your GP currently prescribing medication for your blood pressure or heart condition?
Do you know of any other reason why you should not take part in physical activity?

If you have answered ‘YES’, please sign this box to confirm that you have sought and obtained medical clearance from your GP and that your GP has confirmed it is safe for you to take part.

If you answered 'NO' to ALL the questions: It is reasonably safe for you to participate in physical activity, gradually building up from your current ability level.

IMPORTANT PLEASE READ BEFORE SIGNING: All information shared on this form is kept private and confidential. Personal information is only kept for the duration of time that you participate as a client of KBSC, and you are required to complete a new PAR-Q form every 12 months,  or if anything has changed regarding your health that would affect your participation training provided by KBSC. Data is securely stored electronically and is kept in line with EU General Data Protection Regulation (EU GDPR) and Data Protection Act 2018 guidelines on our secure, password protected database and no third parties have access to any private information you have shared with us on this PAR-Q form.   We will never share any of your personal data with any third-party without your knowledge, consent and agreement. We will retain the information for only as long as necessary and as outlined in our privacy policy, after which point your stored PAR-Q information will be deleted from our database.  By signing this form, you confirm that you have read, understood and consent to us storing and handling your personal information (contained in this PAR-Q form) as outlined above.
